Orange-A-Thon Walk/Run/Bicycle & Fund Raiser

Saturday, October 12, 2013 event:

YES, Inc. is helping to raiser funds for public/private/charter schools through donations and providing a fun healthy familiy activity on the walk/run/bicycle path along the Metro Orange Line.

Orange-A-Thon walk/run/rides, are:

  • 25 miles "Fun Ride" from Chatsworth to Balboa Park with a stop at the Encino Velodrome. Registration begins 8:00 am, ride starts 8:30 am
  • 8 miles "Family Run/Ride" from Chatsworth Depot to Woodland Hills and back. Registration begins 9:00 am, ride starts 9:30 am
  • 1.5 to 3 miles "Young Families Walk/Run/Ride from Chatsworth Depot to Nordoff and back. Registration begins 9:30 am, ride starts 10:00 am 
  •  Healthy and Active Fair from 9:00 am - 1:00 pm
  •  Awarding the Orange-A-Thon participants prizes and bicycle raffles, starting at 11:00. Winners do not need to be present.

All children ages 1-14 must be accompanied by an adult. All must wear a helmet while riding a bicycle. All riders/walkers may begin as soon as they are register or checked-in.

Pre-Registration: Ride Tickets can be purchased from participating schools; Police Activities League  or pre-register online before October 10, 2013, for:

  • Adult $5
  • Teen $5 ages 15-17
  • Child $5 ages 6-14
  • Young Child FREE ages 1-5 (But still needs to pre-register)

Registration on Ride Day - CASH ONLY:

  • Adult $10 
  • Teen $8
  • Child ages 6-17 $5
  • Young Child ages 1-5 FREE (But still needs to register)

Direction to the Start/Finish:  The Chatworth Train Depot, 10047 Old Depot Plaza Road, Chatsworth, CA sits between Devonshire Blvd. and Lassen Avenue about one mile East of Topanga Blvd.  Topanga Blvd. and can be access from the 118 Freeway via Topanga Blvd. 6 miles West of the 405 Freeway.

Three ways to register for the Orange-A-Thon:

  1. To pre-register online complete the Orange-A-Thon Walk/Bike form below - "submit form" and to register for the event click on "Donate".  Online pre-registration closes October 10, 2013 at midnight.
  2. If you are unable to pay online with a credit card or PayPal, download and mail the Orange-A-Thon Pre-Registration Form English/Spanish (see attachments below), complete and mail with payment. Or email: info@yesports.org for an Orange-A-Thon Walk/Bicycle Pre-Registration form. Complete the form and mail in with a check by October 7, 2013.
  3. Participants can register on event day and pay with cash only.  

 

2013 Orange-A-Thon Walk/Bike - Pre-registration and Donation Form

Please complete all required fields for the walk/bike activity of choice. All required fields must be completed and donation made by PAYPAL. If unable to make payment by PAYPAL contact info@yesports.org or 818-292-0779 with questions and to receive a mail-in form with a personal check by October 10, 2013.

Start/Finish is at the Chatsworth Train Depot, 10046 Old Depot Plaza Road, Chatsworth, CA 91311. Parking in the North lot off of Devonshire Blvd.

All must register to participate. All adults must be accompanied with a child, ages 1-15. Youth ages 16 and up need not be accompanied by an adult, but needs a parent signature. Wearing a helmet when riding is mandatory. All walk/bike activities require a donation. Please sign waiver and check off the walk/ride you plan to do below:

ACCIDENT WAIVER AND RELEASE OF LIABILITY. I acknowledge that this athletic event is an extreme test of a person’s physical and mental limits and carries with in the potential for death, serious injury and property loss. The risks include, but are not limited to, those caused by terrain, facilities, temperature, weather, condition of athletes, equipment, vehicular traffic, actions of other people including, but not limited to, participants, volunteers, spectators, coaches, event officials, and event monitors and/or producers of the event and lack of hydration. I realize that liability may arise from negligence or carelessness on the part of persons or entities being released, from dangerous or defective equipment or property owned, maintained or controlled by them or because of their possible liability without fault. I certify that I am physically fit, have sufficiently trained for participation in the event and have not been advised otherwise by a qualified medical person. I acknowledge that this Accident Waiver and Release of Liability (AWRL) form will be used by the event holders, sponsors and organizers, in which I may participate and that it will govern my actions and responsibilities at said events. In consideration of my application and permitting me to participate in this event, I hereby take action for my self, my executors, administrators, heirs, next of kin, successors, and assigns as follows: (A) Waive, Release and Discharge from any and all liability for my death, disability, personal injury, property damage, property theft or actions of any kind which may hereafter accrue to me or my traveling to and from this event. THE FOLLOWING ENTITIES OR PERSONS: Youth Educational Sports, Inc., their directors, officers, employees, volunteers, representatives, and agents, the event holders, event sponsors, event directors, event volunteers; (B) Indemnify and Hold Harmless the entities or persons mentioned in this paragraph from any and all liabilities or claims made be other individuals or entities s a result of my actions during this event. I hereby consent to receive medical treatment which may be deemed advisable in the event of injury, accident and or illness during this event. I understand that at this event or related activities, I may be photographed. I agree to allow my photo, video or film likeness to be used for any legitimate purpose by the event holders, producers, sponsors, organizers and or assigns. This AWRL shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law. I hereby certify that I have read this document; and I, understand it’s content. PARENT GUARDIAN WAIVER FORM FOR MINORS (Under 18 years old) The undersigned parent and natural guardian or legal guardian does hereby represent that he/she is, in fact, acting in such capacity and agrees to save and hold harmless and indemnify each and all of the parties referred to above from all liability, loss, cost, claim or damage whatsoever which may be imposed upon said parties because of any defect in or lack of such capacity to so act and release said parties on behalf of the minor and the parents or legal guardian. I AGREE TO WEAR A HELMET WHILE RIDING.
